Agree and consent to the Terms of Service, Privacy Policy and Data Sharing Agreement of Elevate Washington.
Understand that you are creating an Elevate Washington User Account (“Account”).
Agree that you are 18 years of age or older and that the information you provide as part of the Account creation process is true, current, and complete.
Understand that School’s Out Washington may remove your Account, for any reason, in its sole discretion, including that School’s Out Washington may immediately terminate or suspend your Account if you violate any part of this Agreement, Elevate Washington’s Terms of Service, Elevate Washington’s Privacy Policy, or the Data Sharing Agreement you must agree to each time you submit data to Elevate Washington. Upon termination of your Account, you will no longer be able to access your Account.
Understand that any information provided by you in relation to an Account may be edited by you at any time. You further understand that School’s Out Washington may modify your Account information pursuant to your reasonable request to update or change any information so long as such modifications are true and current, or if such information is incorrect or in violation of this Agreement, any third party’s intellectual property, or any applicable laws or regulations.
Understand that School’s Out Washington reserves the right, at any time and in its sole discretion, to request documentation to determine or confirm Account ownership or related information. Such documentation may include, but is not limited to, a scanned copy of your business license or a government issued photo identification. In the event of a dispute regarding Account ownership, School’s Out Washington reserves the right to determine, in its sole judgment, the rightful Account Owner and transfer an Account to the rightful owner. If School’s Out Washington is unable to determine the rightful Account Owner, School’s Out Washington reserves the right to temporarily disable the Account until resolution is reached between the disputing parties.
Understand that you are responsible for keeping your Account and password secure and confidential. Furthermore, you are responsible for any and all activities that occur under your Account. You agree to immediately notify School’s Out Washington of any unauthorized use of your Account or any other breach of security of which you become aware by emailing info@elevatewashington.org. You are responsible for taking precautions and providing security measures best suited for your situation and intended use of Elevate Washington. School’s Out Washington will not be liable for any loss or damage arising from your failure to maintain the security of your Account or password.
Understand that to create an Account, you will be required to provide personal information. School’s Out Washington may use this personal information to provide service and information to you. Your personal information will only be visible to School’s Out Washington and will not be shared with any other party subject to the exceptions identified in Elevate Washington’s Privacy Policy. You further understand that although School’s Out Washington has safeguards in place to prevent unauthorized access, use, or disclosure of personal information in our possession, we cannot guarantee that your personal information will be secure.
Understand that you may cancel your Account at any time by emailing info@elevatewashington.org and then following the specific instructions provided to you by School’s Out Washington. Upon termination of your Account, you will no longer be able to access your Account.
Agree that you are authorized to submit data to Elevate Washington on behalf of the Organization you represent. You further agree that all submissions of data to Elevate Washington are subject to Elevate Washington’s Terms of Service and Privacy Policy, and the Data Sharing Agreement you must agree to each time you submit data to Elevate Washington.
Understand that administrators of and users associated with an Organization profile are able to view data and run reports on their Organization and all associated Sites and youth development program (“Program”) profiles. Administrators of an Organization profile also are able to view aggregate data and create reports based on these data.
Understand that School’s Out Washington will use the email address provided by you during account creation as the primary method of communication with you.
